In an effort to help us meet your medical needs and make your visit go smoothly, Primary Healthcare has compiled some basic information for our first-time patients. Please read below and then visit our Patient Forms page.

For your first visit, please arrive at least fifteen minutes prior to your scheduled appointment.

Please bring picture identification, applicable health insurance information (card), and any completed paperwork that may have been sent to you by our staff or downloaded from our website prior to your appointment.

During your first visit, you may be asked questions about your health history, so that the medical provider can better assess your condition.

Payment is due at the time of service (this includes any co-pay amounts for those covered under health insurance).

Established Patients

At every visit, please update the medical staff your correct pharmacy and contact information, so there is no delay in receiving your prescriptions and updates on your medical information.
