Think of a primary care doctor like the general contractor of a project. A general contractor has all of the project information and works as a central contact to keep the entire project running smoothly. As needed, the contractor hires specialists (like an electrician and plumber) to handle the part of the jobs they specialize in. Your PCP (Primary Care Physician) is the first person you contact when you have a health question or issue. If needed, your PCP will refer you to specialists. After you see a specialist, your Primary Care Doctor will continue to be your advocate and work with you to resolve your health issues.

The field of primary care, in the United States, which covered general internal medicine as well as general pediatrics, resulted in two reports from the Institute of Medicine (IOM) (Donaldson et al. 1996; IOM 1978). These reports defined primary care as “the provision of integrated, accessible health care services by clinicians who are accountable for addressing a large majority of personal health care needs, developing a sustained partnership with patients, and practicing in the context of family and community.” This definition is consistent with at least two international reports (WONCA 1991; World Health Organization 1978) and has been used to measure the four main features of primary care services: first-contact access for each need; long-term person- (not disease) focused care; comprehensive care for most health needs; and coordinated care when it must be sought elsewhere. Primary care is assessed as “good” according to how well these four features are fulfilled. For some purposes, an orientation toward family and community is included as well (Starfield 1998).
